Add injection class for spinal injections.

Using coordinates ML (lateral to midline) and depth.

There are requestor comments that specifies the vertebrae that would probably be nice to have too, but that might be an ingest issue rather than a schema one.

This might fit into the new Injection class with a CoordinateSystem in Surgery, tbd
