Help me on the finding this Migrate Azure PowerShell script command in command palette #116

Description of the issue
Read the documnet [https://learn.microsoft.com/en-us/powershell/azure/migrate-from-azurerm-to-az?view=azps-10.2.0#option-3-migrate-your-scripts-in-visual-studio-code-with-the-azure-powershell-extension]

I have downloaded the Azure powershell Tools extension. Next step says Start the migration by opening the command palette Ctrl+Shift+P and select Migrate Azure PowerShell script.

But I cant find this "Migrate Azure PowerShell script" in command palette.

MicrosoftDocs/azure-docs-powershell#2693 had the same issue

the command palette "Migrate Azure PowerShell script" had been removed long ago, I should remove this part in doc and provide more accurate description.
