[User] Sing-in/up option

Give the option to the user to Sign In or Sign Up to our webpage.

We need to create the corresponding database table(s).
The Sing Up form will have the following field:
a. Username (required)
b. password (required)
c. Full Name
d. email (required)
e. phone number (required)
f. Wanted Appointment type (skype, phone call, Viber)
g. Skype Username (required is skype is selected)

The Sign In form will have the following fields:
a. Username (required)
b. Password (required)
c. Forget my password - link (required)

The 'Forget my password' form will have only the Email field.
