Can't unsubscribe from private communication lists

Description

Reported on behalf of North Point Ministries. In previous versions of Rock prior to one-click unsubscribe, the unsubscribe link at the bottom of emails would allow you to unsubscribe from communication list used to send the specific email whether or not they were public. After one-click unsubscribe, private email lists are not shown and there is no way to unsubscribe from the private list. The only options are "No Bulk Emails" or "No Emails." The reason I would classify this as a "bug" is that the desired behavior is exactly what one-click unsubscribe will do if the transport chooses to show the one-click unsubscribe option. There is functionality provided by one-click that is not provided by the footer unsubscribe. If a private communication list was used, I would expect to see the option to unsubscribe from just that list on the unsubscribe page.

Actual Behavior

If a private communication list is used and "Bulk" is selected, the footer unsubscribe is displayed in the email. When clicked, it goes to the unsubscribe page, but no list is shown from which you can unsubscribe. Only the "Email Allowed," "No Bulk Email" and "No Email" option are available.

Expected Behavior

If any Communication List was used to send the email, I would expect to see only that Communication List shown with the value being unchecked so the unsubscriber can simple click OK.

Steps to Reproduce

1. Go to Admin -> Communication -> Communication Lists
2. Create a test Communication List that is Active but has Public unchecked
3. Add a person to that list
4. Use that Communication List to send to the recipient and mark as Bulk.
5. Open the email and click the Unsubscribe link
6. The Communication List is not available on the screen to unsubscribe
