warns.js => moderator definition can break if no moderator in server [KNOWN]

Anywhere where the moderator_id is queried into a return for an embed warns output, if the moderator in question is not in the guild in question, the bot will break. I can't be bothered to fix it right now, even though it's an easy fix, so just putting it as a known issue instead.
