macOS currently refuse to open it as it's not signed and last commit was 2 years ago

Just macOS changed behavior for unsigned plugins:

Open system settings → Privacy & security → Security and there should be banner allowing you to approve this plugin.

Thanks! This doesn't seem to work in macOS Sequoia though AFAICT ...

On macOS Sequoia the support for older Quicklook genarator is removed: https://mjtsai.com/blog/2024/11/05/sequoia-no-longer-supports-quicklook-generator-plug-ins/
