When creating a new issue, please include:

The version of calibre you are using
Calibre's language setting (available in Preferences | Interface | Look and Feel | Choose language)
Your operating system, and 32- or 64-bit, if applicable. For example:
- Windows 7, 64-bit
- OS X 10.8.4
- Ubuntu 12.04 32-bit
The version of the plugin installed
The specific steps to recreate the problem
A description of the problem
