Create Design Doc for GitHub Action PR CoP Assignment Checker

Description

We have noticed that when a PR review is being reviewed AND has comments left for feedback, it removes the CoP assignment label. This can cause an issue because the PR may be lost if the Tier 2 engineer doesn't jot down that it is pending commentary. As a result, we need to come up with a solution to ensure PRs are done efficiently and aren't lost.
