Sometimes email templates are interchanged. Example:

  1. Create template for accepted and save
  2. Create template for rejected and save
  3. Then sometimes, the accepted one will show up for the rejected ones, or vice versa. It may also happen to pending emails.

Email templates have also been reported to be lost after a while.

The cause might be in the API
