Change custom form field identifiers from camel case to snake case

For some reason(likely frontend centric approach), field identifiers were denoted in camel case instead of snake case. Once #7071 is complete and relevant code is removed from frontend, this will be changed on server and a migration will be created for the same

I think the changes has been made. all identifiers are in snake case. so maybe you should close this issue. or if its yet not fixed maybe i can work on this.

@AtmegaBuzz we are still figuring out how to progress with it. But there is already an open PR with relevant changes. I suggest you pick some other issue. Thanks.
