Inconsistent highlighting of bracketed terms

In the following file:

[foo] bar [baz] quux
hi [foo] bar [baz]
hi \[foo\] bar \[baz\]

all items in brackets get highlighted (underlined) except the first baz and the second foo. I'd expect uniform behavior on the first two lines, and no highlighting on the last.

Actually, in this file, no highlighting should be done, because there are no reference definitions. This is an ongoing issue.

I just fixed the escaped \[ issue in master.
