Admin Data Completeness report formatting - data source selection

Situation - Admin Data Completeness report shows data from different forms in one query result. This is confusing and does not summarize the data well as different forms have different numbers of days. e.g., some forms have 3 days so how is the day 4 col treated in this case.

Proposed Solution - Add a dropdown menu to select from the different admin forms. Upon selecting the admin form of choice the report would show the appropriate number of day columns
