Update Habitat Quality output list

Habitat Quality

• Adds rarity_c_[Suffix].csv and rarity_f_[Suffix].csv to Habitat Quality model output list (see natcap/invest #721).
• Removes _out from existing items on HQ output list (since _out is not included in the filenames actually generated by the model).
• Removes [Workspace]\output folder heading from HQ output list (because the model does not generate anoutput subfolder).
• Fixes broken equation links in HQ output list.

The diff makes it look like I completely rewrote the output section, but I didn't. The changes I made are limited to what's listed above.

Miscellaneous

• Fixes a couple of tiny typos in the Spanish-language version (ed => de and lq => la).
• Updates the sample data command in the README to match what's defined in the Makefile (and what actually works currently).
• Updates contributors.
• Some whitespace changes were applied automatically.
