Add a contributor ladder requirement

Per #173, this adds a requirement to have a contributor ladder to the entry requirements for the incubating stage.

This also makes it explicit that projects should document their current list of maintainers.

It is preferred that extensions of existing OpenSSF projects collaborate with the existing project rather than seek a new project.
