chore: move tracing-opentelemetry into the repo root

Currently, all the code and other files are in a `tracing-opentelemetry` subdirectory of the repository, which is an artifact of the code previously living in the `tokio-rs/tracing` monorepo. This branch pulls all the files in this directory out into the root of the Git repo. No functional changes.
